Community Services

Welcome to our Community Services section, the next generation of Anne Carlsen Center care. For decades, our Jamestown campus was the only location in the state where we could fully meet the needs of individuals with disabilities. That changed in June 2008 with the launch of our Community Services which created greater access to all of the Center’s expertise and care. We offer In-Home Supports as well as Community Day Supports, helping individuals enjoy lives of dignity and independence. These programs also provide guidance for those in an assisting capacity.

We still offer exceptional residential services on our ACC Campus. But we are also meeting a growing need for services and supports in communities across the state. Our Community Services do not offer the same residential supports available at our Jamestown Campus. But we’re making it possible for more children to live in their natural family environments and more adults to become active and productive members of their communities.

Through our Community Services, we will provide space for therapy assessments, team meetings and group trainings without the hassle and time wasted of a cross-state drive.

In-Home Supports

In-Home Supports assist individuals and their families, creating a stronger network and a greater chance of success in all areas of life for individuals with disabilities.

It takes a strong support system to help a person live a healthy, meaningful life.

Through In-Home Supports, the Anne Carlsen Center will help create your child’s foundation of learning. The focus will not center on just the individual, but also factors in the roles of parents, family members and caregivers, among others. Each will learn skills and routines that support the plan we’ve crafted for your child.

The Center’s programs are customized to your family and address your specific needs. We help track an individual’s development through a series of goals and objectives.

Areas where In-Home Supports assist an individual’s learning:

  • Attending to personal care
  • Preparing snacks and meals

  • Dressing and hygiene skills

Your child's plan will:

  • Focus on skills development through implementation of goals and objectives
  • Show progress, or the attempt at progress in development of skills
  • Maintain continuity with team planning across all settings
  • Establish and monitor skill development objectives in behavioral, life and social skills
  • Include dressing and hygiene skills

This program is intended to grow and change with an individual’s life. Our goal is to help each individual reach his or her ultimate potential. Through this program, the seeds of an independent life will be planted.

Planning grocery lists and carrying out shopping trips are one area covered in the Community Day Supports program.

The Anne Carlsen Center believes a key to becoming a well-rounded individual is creating and maintaining an active relationship with one’s surroundings.

Through the Anne Carlsen Center’s Community Day Supports Program, we provide access and opportunities for individuals aged 21 and older to engage in their community. Socializing through new and different networks opens doors to help individuals focus on specific life areas.

We want to help create a valuable community member -- someone who embraces the challenges of everyday life. Our Community Day Support programs are tailored to the people we serve. ACC works with those closest to each individual to develop a clear idea of the support our clients require to live life to its fullest.
